1. I understand that I should consult with my physicians or healthcare providers prior to participating in the classes as and when necessary. 

2. I will notify my instructor of illness or injury of myself (and my baby - for Baby & Me) before each class. I understand that I am responsible for the health, wellbeing and safety of myself (and my baby - for Baby & Me) when participating in the classes.

3. Even with clear instruction and no underlying health condition, there is the possibility of injury or potential health risk involved in participating in the classes. I HEREBY VOLUNTARILY AND FOREVER RELEASE, WAIVE, AND DISCHARGE the instructor (Angora Ngai) OF AND FROM ALL claims, damages, demands, actions or causes of action, whatsoever and however caused, arising by reason of my participation in the classes.
